Appointments

Please request appointments by calling the office directly or making requests on the patient portal link. We understand occasional circumstances prevent you from keeping your scheduled appointment. Please notify our office 48 hours in advance if you are unable to make the appointment. Our scheduling coordinator will do their best to re-schedule you promptly. There is a cancellation policy of $100 for appointments cancelled later then the recommended 48 hour grace period.

Insurance and Billing

At present, Dr. Sadiq is not in-network with insurance. However, if out-of-network benefits are present, we can provide an invoice for reimbursement purposes.  

Payment Options

We accept checks, cash or credit cards. As an out of network provider for insurance we offer invoices to file to your insurance for your reimbursement.
